How do I know if I qualify for PACE enrollment?
PACE Southeast Michigan offers an opportunity for frail seniors to receive the care they need in their homes (and our day health center), as long as possible. However, PACE eligibility is determined by specific PACE enrollment criteria as dictated by the State of Michigan and the Centers for Medicare and Medicaid Services.

To be eligible for PACE enrollment, you must meet the following criteria:
You are 55 years of age or older.
Eligible for nursing home level of care as determined by the State of Michigan
Live in the PACE Southeast Michigan designated service area (see below)
Must be safe in the community with the support of PACE Southeast Michigan

Are people who do not qualify for Medicaid eligible for PACE enrollment?
Yes. If a person meets the income and assets limits to qualify for Medicaid, the program pays for a portion of the monthly PACE premium; Medicare pays for the rest. Individuals who do not qualify for Medicaid are responsible for the portion of the monthly premium Medicaid would pay. PACE program staff can help determine a person's Medicaid eligibility.
How much does PACE Southeast Michigan cost?
Your cost will depend on your Medicare and Medicaid eligibility:
If you are eligible for both: There is no out-of-pocket cost.
If you do not qualify for Medicaid: There is a monthly fee.
Because PACE Southeast Michigan provides and is responsible for all of your care, you may be financially responsible for any care you receive outside the program that is not approved by the PACE Southeast Michigan Interdisciplinary Team (except in an emergency).
