Do I qualify for PACE?

Our comprehensive program offers an alternative to nursing homes, while also allowing peace of mind for family and caregivers. PACE Southeast Michigan provides older adults the all-inclusive, coordinated care they need to age in the homes and communities they love.

You may qualify for PACE Southeast Michigan if you meet the following criteria:

  • 55 years or older

  • Able to live safely in the community with the support of PACE Southeast Michigan services

  • Meet the State of Michigan nursing facility level of care determination

  • Resident of our PACE Southeast Michigan designated service area

Because PACE Southeast Michigan provides and is responsible for all of your care, you may be financially responsible for any care you receive outside the program that is not approved by the PACE Southeast Michigan Interdisciplinary Team (except in an emergency).
