PACE Southeast Michigan offers a unique line-up of coordinated care to help you stay in the home and community you love.

 

Spend time at our day center filled with caring staff who feel like family and receive medical services like primary care, dental, vision and physical therapy offered at our facility or in your home. We'll even pick you up and drop you off at the end of the day.

What you should know about PACE Southeast Michigan

PACE Southeast Michigan provides comprehensive care. We use Medicare, Medicaid, and private funds to cover all your medically necessary care and services. This eases the burden and worry for you of what is covered and how much you may have to pay.

The focus is on you. As part of the PACE Southeast Michigan family, you have a team of health care professionals to help you make important health care decisions. Your team is experienced in caring for those with your level of need. They usually care for a small number of people. This way, they get to know you, your living situation, what you like and don’t like! You, your family, and your caregivers have a say as the team develops and updates your plan of care and your goals in the program.
