What Is PACE?

PACE offers preventive care and comprehensive support for eligible seniors.

PACE Southeast Michigan is a Program of All-Inclusive Care for the Elderly (PACE). PACE Southeast Michigan uses Medicare, Medicaid and private funds to cover all of your medically-necessary care and services.

The PACE Philosophy:
Complete Care

PACE Southeast Michigan is committed to helping you live in the community and age in your own residence. To meet this goal, we focus on preventive care. Although all people who enroll in PACE are eligible for nursing home care, only 7 percent of PACE participants across the United States live in nursing homes.

Our Mission

PACE Southeast Michigan is a unique health plan and care provider, committed to keeping older adults with challenging healthcare conditions in their home, by caring for their medical, physical, social, and spiritual needs.

Our Values

Extraordinary Care
Collaborative Teamwork
Inspired Achievement
Accountability
Adaptive Growth

PACE services are designed to meet your individual needs.

  • PACE Southeast Michigan provides care and services in you home, community, and our day health centers throughout Wayne, Oakland, and Macomb counties.

  • PACE Southeast Michigan participants receive most of their care from our expert staff at our centers.

  • We provide transportation to and from our centers and all medically-necessary appointments.

One source for all of your care.

When you enroll in the PACE Southeast Michigan program, we provide and are responsible for all of your care, which is provided to you at no cost. However, you may be financially responsible for any care you receive outside the program that is not approved by the PACE Southeast Michigan interdisciplinary team (except in an emergency).
